ABSTRACT

Objective:To explore the intervention effect of nursing guided by self-efficacy theory on postpartum breastfeeding of patients with gestational diabetes mellitus.Methods:A total of 140 patients with gestational diabetes mellitus in Qingdao Women and Children ′s Hospital were selected as study subjects by convenient sampling method. They were divided into two groups according to the time of diagnosis. Seventy patients recruited from August to September 2019 were named as the control group, while those 70 patients from May to June 2020 were set as the intervention group. The control group took routine care and the intervention group received breastfeeding intervention based on self-efficacy theory. The rate of delayed onset of lactogenesisⅡ(DOLⅡ), breastfeeding knowledge, exclusive breastfeeding rate and breastfeeding self-efficacy in different time were compared. Results:The rate of DOLⅡ in the intervention group was 11.43% (8/70), which was lower than 25.71% (18/70) in the control group, the difference was statistically significant ( χ2=4.72, P<0.05). The score of breastfeeding knowledge in the intervention group at postpartum day 3 was 16.0 (15.0, 17.0) , which was higher than that in the control group 14.0(13.0, 16.0), the difference was statistically significant ( Z=-3.55, P<0.05). The rates of exclusive breastfeeding at postpartum day 3, month 1 and month 3 was 78.57% (55/70), 68.57% (48/70), 61.43% (43/70) in the intervention group and 61.43% (43/70), 45.71% (32/70), 37.14% (26/70) in the control group, the differences were statistically significant ( χ2=4.90, 6.60, 8.26, all P<0.05). The total scores of breastfeeding self-efficacy before intervention, 3 days, 1 month and 3 months postpartum were (48.34 ± 5.37), (50.00 ± 6.06), (52.54 ± 5.41), (52.92 ± 5.62) points in the control group and (48.96 ± 4.30), (53.32 ± 5.94), (56.91 ± 5.03), (59.11 ± 4.70) points in the intervention group. The total score of breastfeeding self-efficacy of patients in the two groups was analyzed by repeated measures of variance, and the differences in time effect, between-group effect and interaction effect were statistically significant ( Ftime=81.61, Fbetween-group=31.31, Finteraction=10.23, all P<0.05). Conclusions:Breastfeeding nursing intervention based on self-efficacy theory can improve the knowledge level of breastfeeding in patients with gestational diabetes mellitus, reduce the rate of DOLⅡ, enhance the sense of self-efficacy of breastfeeding and promote exclusive breastfeeding.

ABSTRACT

Objective:To retrospectively analyze the urodynamics quality in Southwest China, and find out the main issues of urodynamics quality in Southwest China and try to find out the improvement ways.Methods:In this study, a two-stage sampling method was used.In the first stage, 10 medical institutions in Southwest China were selected by cluster sampling from March to June, 2020.In the second stage, according to the development of UDS in Southwest China, the sample size estimation formula was adopted, and the loss of follow-up rate in reports extraction was considered, the initial sample size was 350. As the workload of UDS in the 10 medical institutions involved in the study was equivalent, 35 urodynamics traces from each medical institution were selected. The initial samples should also meet the inclusion criteria: ①patients with clear medical history and complete clinical data; ②UDS traces were clear; ③UDS system was water filled system; ④age>18, and 150 urodynamic traces were included in the final study. We evaluated the quality of enrolled urodynamics traces, and the quality evaluation standard according to the guidelines established by the International Continence Society (ICS). The evaluation conducted by two independent urologist with more than 10 years working experience. Artifacts were divided into non-technical artifacts: abnormal abdominal pressure changes, urine volume <150 ml when did the uroflow test, and technical artifacts: non-standard zero setting, fail to record all urodynamics parameters, baseline drift, catheter displacement, misjudgment of detrusor physiological contraction and detrusor overactive in voiding phase, misjudgment between detrusor overactive and bladder low compliance in filling phase.Results:non-technical artifacts: 32 cases were found abnormal abdominal pressure changes (21.3%), 21 cases (14.0%) were found when did the uroflow test the urine volume <150 ml, and technical artifacts: Non-standard zero setting in 28 cases (18.7%), fail to record all urodynamics parameters in 8 cases, baseline drift in 16 cases, catheter displacement in 9 cases and misjudgment of detrusor physiological contraction and detrusor overactive in voiding phase in 12 cases, misjudgment between detrusor overactive and bladder low compliance in filling phase in 24 cases (16.0%).Conclusions:At present, the urodynamics quality in Southwest China need to be improved. The main issues were that the operator didn’t obey the basic operation and quality control process, and the operator did not have enough basic knowledge of urodynamics. It can be improved by strictly carry out the operation standard of UDS, identifying and correcting artifacts in time, and promoting the standardized urodynamic training courses.

ABSTRACT

Diabetic cardiomyopathy occurs in diabetic patients and is different from hypertensive heart disease, coronary atherosclerotic heart disease, and other cardiac abnormalities. The main clinical symptoms are systolic and diastolic cardiac dysfunction, myocardial fibrosis, congestive heart failure, and angina pectoris. As one of the main complications of diabetes, its incidence and fatality rates have been on the rise year by year. However, modern medicine still fails to figure out its pathogenesis and no specific drug is available, which has seriously affected the survival and quality of life of patients. Cardiomyocytes contain a large number of mitochondria, which participate in cardiac energy metabolism and other biological activities and occupy an important position in the development of diabetic cardiomyopathy. Mitochondrial quality control mainly involves mitochondrial oxidative stress, mitochondrial dynamics, mitochondrial autophagy, and intracellular calcium regulation, which is an important condition for stabilizing the normal mitochondrial structure and exerting normal mitochondrial functions. In recent years, the efficacy of traditional Chinese medicine in intervening in mitochondrial quality control through multiple angles, pathways, and targets to affect the structure and function of myocardial mitochondria and significantly improve the clinical symptoms of patients with diabetic cardiomyopathy has attracted wide attention from scholars. Therefore, this paper reviewed the experimental studies and/or clinical observations concerning the treatment of diabetic cardiomyopathy with effective compounds of Chinese herbs and/or Chinese herbal compounds in the past ten years to further explain the pathogenesis of diabetic cardiomyopathy, clarify the regulatory mechanism of traditional Chinese medicine in mitochondrial quality control, and summarize the scientific connotations and shortcomings of traditional Chinese medicine in the treatment of diabetic cardiomyopathy, hoping to provide certain ideas and methods for further clinical application of traditional Chinese medicine in the treatment of diabetic cardiomyopathy.

ABSTRACT

OBJECTIVE@#To explore the genetic basis for a fetus featuring growth restriction and validate the effectiveness of a novel noninvasive prenatal testing (NIPT) technique for the detection of chromosomal microdeletions.@*METHODS@#Next-generation sequencing(NGS) and fluorescence in situ hybridization(FISH) were used to analyze the DNA of the fetus. Conventional G-banding was used to analyze the karyotypes of the fetus and its parents. High-throughput sequencing was used to analyze free fetal DNA.@*RESULTS@#NGS analysis has revealed a 4.88 Mb deletion at 15q11.2-q13.1 region in the fetus, which has a 99% overlap with the critical region of Prader-Willi syndrome (Type 2) and Angelman syndrome (Type 2) and encompassed critical genes including SNRPN and UBE3A. NIPT also revealed a 4.6 Mb deletion at 15q12, which was consistent with the results of fetal cord blood and amniotic DNA testing. FISH assay has confirmed the result of NGS. By karyotying, all subjects showed a normal karyotypes at a level of 320~400 bands.@*CONCLUSION@#It is quite necessary to carry out genetic testing on fetuses showing growth restriction. NIPT for fetal chromosomal microdeletions/microduplication syndromes is highly accurate for the diagnosis of Prader-Willi/Angelman syndrome.

ABSTRACT

Recently, early reperfusion therapy has become the optimal treatment for acute ST-segment elevation myocardial infarction (STEMI). One of the most effective reperfusion strategies is percutaneous coronary intervention (PCI). Depend on the time of chest pain onset, transfer mode of patients and treatment capacity of hospital, the methods of PCI are different, including primary PCI, rescue PCI and facilitated PCI. How to seize the optimal time of intervention therapy and reduce the time interval from symptom onset to reperfusion to rescue the dying myocardial cells are the key for improving prognosis, reducing mortality and decreasing major adverse cardiovascular events (MACE) of patients.

With the advancement of manned space science and technology,the scope of human exploration into outer space has been gradually expanded.The time and distances that astronauts flight on-board are becoming longer and longer.However,long-term microgravity can negatively affect astronauts' physical and psychological aspects,thus influencing their health and leading to the failure of work tasks,and even threatening their lives.This paper reviews USA,Russia and Europe research results in the field of astronauts on-board physical training methods and health protection measures,with the purpose of learning from their successful experience.Overall,it is suggested that for cross complementation in sport science,we should make the introduction of advanced and successful training systems to solve problems existing in astronauts on-board training,ensuring the maintenance of health.

ABSTRACT

Objective To investigate the clinical efficacy of alprostadil combined with butylphthalide on patients with acute ischemic stroke and its effect on hs-CRP.Methods Eighty-five patients with acute ischemic stroke admitted into our hospital were divided randomly into observation group (n =42) and control group (n =43).Patients in control group were treated with alprostadil,and those in observation group were treated with alprostadil combined with butylphthalide.The neurologic impairment score (NIHSS),ADL score,hs-CRP,coagulation function,and clinical efficacy in the two groups were observed and compared.Results After 14 days of treatment,the NIHSS and ADL score in both groups were improved,and NIHSS in observation group were much lower than control group,ADL were much higher,and hs-CRP were markedly lower than that of control (P < 0.05).And the coagulation function indexes in both groups were obvious lowered,and DD and FIB in observation group were conspicuously lower than those in control group,and PT were much higher (P < 0.05).In addition,observation group with effective rate of 90.5% were significantly higher than that of control group with 72.1% (P < 0.05).Conclusion Compared with only alprostadil treatment,alprostadil combined with butylphthalide had a better capability of treating patients with acute ischemic stroke by improving coagulation function and neurologic function,and inhibiting levels of hs-CRP.

ABSTRACT

Objective To explore the effects of trendelenburg position on intraocular pressure during robotic-assisted radical cystectomy and to analyze its influencing factors.Methods Forty-one patients scheduled for roboticassisted radical cystectomy were included.Perioperative IOP measurements were performed as follows:10 min after anesthesia induction with supine position(T1),10 min after maintaining trendelenburg position(T2),1 h,2 h and 3 h after maintaining Trendelenburg position (T3,T4,T5),10 min after supine position (T6),2 h after supine position(T7).Gender,age,BMI,duration and angle of trendelenburg position,and blood loss were recorded.Results Mean IOP after trendelenburg position was 24.88±2.61 mmHg,which was higher than normal.Mean IOP in Trendelenburg positioning were significantly higher than those at T1,T6,T7,and IOP increased with time.The influencing factors of IOP included duration and degree of Trendelenburg position,and blood loss.Conclusion IOP increased in patients undergoing robotic-assisted radical cystectomy in Trendelenburg position.

ABSTRACT

Objective To observe the clinical efficacy of electroacupuncture (EA) at the three groups of Stomach- Meridian acupoints in treating chronic superficial gastritis (CSG) and its effect on serum gastrin level and electrogastrogram, for providing clinical evidence for the experimental result of the three groups of Stomach-Meridian acupoints.Method Sixty-six CSG patients were divided into a treatment group and a control group by using stratified random method, 33 cases in each group. The treatment group was intervened by EA at the three groups of Stomach-Meridian acupoints, while the control group was intervened by EA with ordinary acupoints selection. The intervention was given once a day, successive 6 sessions as a treatment course, for 4 courses in total. The CSG symptoms and syndrome score, serum gastrin content, average amplitude of electrogastrogram, mean waveform frequency, and the percentage of gastric dysrhythmias were observed before and after the intervention, and the clinical efficacies were compared.Result The total effective rate was 90.9% in the treatment group, versus 87.9% in the control group, and the between-group difference was statistically insignificant (P>0.05). The total scores dropped significantly in both groups after the treatment (P<0.05), and there was no significant difference in comparing the total score between the two groups after the treatment (P>0.05). The serum gastrin contents declined significantly in both groups after the intervention (P<0.05), and there was no significant between-group difference in comparing the serum gastrin content after the treatment (P>0.05); the average amplitude and mean waveform frequency of preprandial and postprandial electrogastrogram increased significantly in both groups after the treatment (P<0.05), and the percentage of gastric dysrhythmias dropped significantly (P<0.05); after the treatment, there were no significant between-group differences in comparing the average amplitude, mean waveform frequency and the percentage of gastric dysrhy- thmias of preprandial and postprandial electrogastrogram (P>0.05).Conclusion EA at the three groups of Stomach- Meridian acupoints can effectively improve the symptoms, down-regulate serum gastrin, help gastric motility, and restore gastric rhythm of CSG patients; its efficacy is equivalent to that of the classic acupoints group, i.e. Neiguan (PC6), Zhongwan (CV 12), and Zusanli (ST 36).

ABSTRACT

Objective To explore the relationship between expression of CD142 protein and the promoter methylation in the placenta of severe preeclampsia patients.Methods We assessed 24 patients complicated with severe preeclampsia as case group and 24 normal pregnant women as control group via qRT-PCR, immunohistochemistry and Western blotting for CD142 expression and MSP technology for methylation in CD142 promoter region.Results The relative expression quantity of CD142 mRNA in severe preeclampsia group (1.45± 0.42)was higher than that in normal group (0.25±0.28)(P <0.05).The expression quantity of CD142 protein in severe preeclampsia group (0.857±0.043)was higher than that in normal group (0.248 ±0.035)(P <0.05).The positive rate of CD142 promoter region methylation was lower in severe preeclampsia group than in normal group (29.2% vs .100.0%,χ2 =36.11,P <0.001)while the positive rate of CD142 promoter region unmethylation was higher than that in the latter group (100.0% vs .20.8%,χ2 =29.85,P <0.001).A negative correlation was found between the CD142 promoter methylation level and the expression quantity of CD142 protein (r = -0.909,P <0.05).Conclusion The expression of CD142 protein regulated by the promoter methylation plays a crucial role in the mechanism of severe preeclampsia.

ABSTRACT

Objective Influenza A(H7N9) virus causes a relatively high mortality in humans and therefore it is of great sig-nificance to know its prevalence in China .This article aimed to study the genetic characteristics and evolution of the hemagglutinin (HA) gene of the influenza A(H7N9) virus prevailing in China between 2013 and 2017. Methods We downloaded the HA se-quences of the influenza A ( H7N9) virus prevailing in China between 2013 and 2017 from The Global Initiative on Sharing All Influen-za Data and National Center for Biotechnology Information .Using the bioinformatics software , we analyzed the homology , molecular characteristics , phyletic evolution , and selective pressure of the HA gene. Results The homology of the HA gene of the influenza A ( H7N9) virus and the reference strain was decreasing each year from 2013 to 2017, 99.0%-99.9%in 2013, 98.7%-99.5%in 2014, 98.4%-99.6%in 2015, 76.8%-99.4%in 2016, and 69.9%-98.2%in 2017.Compared with the reference strain , the HA gene of the influ-enza A(H7N9) virus underwent variations in 21 antigenic sites.The variation of N285D was the highest (23%) in 2015 and that of R148K increased yearly,reaching 65%in 2016 and 78.5% in 2017. Phylogenetic analysis showed a concentrative distribution of the influenza A ( H7N9) virus strains on the phylogenetic tree in the same year from 2013 to 2017.Amino acid substitution of T 140A was observed in most of the influenza A ( H7N9) virus strains from Guang-dong in 2013, and the widest distribution of the virus strains was found in 2014.Positive selective pressure site 65 was obtained in the sequence of 2015 using the FEL and IFEL models, but not in the strains of 2016 or 2017. Conclusion Influenza A(H7N9) virus constantly undergoes variation , which has increased the difficulty in its prevention and control .More importance should be attached to observation of the virus and response to its adaptive mutations .

ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of alprostadil combined with salvia ligustrazine in the treat-ment of aged patrents with unstable angina pectoris. METHODS:A total of 150 patients with unstable angina pectoris department of our hospital during Oct. 2011-Mar. 2015 were randomly divided into alprostadil group,salvia ligustrazine group and combination group according to random number table,with 50 cases in each group. Three groups received routine treatment. Alprostadil group additionally received Alprostadil injection 100 μg added into 0.9% Sodium chloride injection 250 mL,ivgtt,qd,on the basis of routine treatment. Salvia ligustrazine group additionally received Salvia ligustrazine injection 10 mL added into 0.9% Sodium chlo-ride injection 250 mL,ivgtt,qd,on the basis of routine treatment. Combination group additionally received constant dose of Al-prostadil injection and Salvia ligustrazine injection. Hemorheological indexes (high shear whole blood viscosity,low shear whole blood viscosity,plasma viscosity,hematocrit,fibrinogen),cardiac function indexes(LVEF,SV,LVEFD,LVST),serum CRP, NO,ET,SOD and clinical efficacies were observed in 2 groups before and after treatment;the occurrence of ADR was compared between 2 groups. RESULTS:Before treatment,there was no statistical significance in hemorheological indexes,cardiac function indexes or serum CRP,NO,ET,SOD level between 2 groups (P>0.05). After treatment,plasma viscosity,the whole blood high and low shear viscosity,hematocrit,fibrinogen,serum CRP and ET levels of 3 groups were decreased significantly,while LVEF,SV,serum levels of NO and SOD were increased significantly,combination group was significantly better than alprostadil group and salvia ligustrazine group,with statistical significance (P0.05). CONCLUSIONS:Alprostadil combined with salvia ligustrazine can effectively reduce the blood viscosity of patients with unstable angina pectoris,improve cardi-ac function and endothelial function,reduce myocardial ischemia injury and show significant therapeutic efficacy and safety without increasing the incidence of ADR.

ABSTRACT

Objective:To study the expressione of autophagy-related protein Beclin-1 and LC3 in cortex and hippocampus of rats after cerebral ischemia reperfusion injury and the efficacy of Edaravone.Methods:Sprague Dawley rats were randomly divided into sham group,model group and Edaravone group.The cerebral ischemia reperfusion model was induced via Zea Longa with blocking the middle cerebral artery of 2 h and reperfusing of 24 h.Animals assigned to sham group were only separated left common carotid artery.Edaravone was injected intraperitoneally at a dose of 1 0 mg/kg at 15 min before reperfusion.The condition of nerve injury of rats was conducted by Neurobehavioral score.The degree of brain injury and success of model were determined based on 2,3,5-triphenyltetrazolium chloride(TTC)staining.The changes of neuron stained by hematoxylin and eosin (HE) in cortex and hippocampus were observed.The expression of Beclin-1 and LC3 was measured by immunohistochemistry.Results:After cerebral ischemia reperfusion the neurobehavioral score of edaravone group was(2.00± 0.67),which was obcviously less than(2.50± 0.53) of model group(P<0.05).The infraction focus and the neuron injury in cortex and hippocampus neurons were also observed in model group,and the edaravone group reduced above expression.The positive rate of Beclin-1 of each group in cortex were (1 1.08± 0.85)%,(33.42± 1.57)% and (25.61± 1.39)%,there was significant difference between model group with sham group and edaravone group (P<0.05).The positive rate of Beclin-1 of each group in hippocampus were (10.34± 0.21)%,(31.82± 1.73)% and (22.74± 1.26)%,there was significant difference between model group with sham group and edaravone group(P < 0.05).The positive rate of LC3 of each group in cortex were (15.33± 0.47)%,(39.72± 1.73)% and (28.53± 1.61)%,there was significant difference between model group with sham group and edaravone group(P<0.05).The positive rate of LC3 of each group in hippocampus were (13.74± 0.37)%,(32.53± 1.43)% and(25.38± 1.23)%,there was significant difference between model group with sham group and edaravone group (P<0.05).Conclusion:The expression of Beclin-1 and LC3 was increased after cerebral ischemia reperfusion.Edaravone may reduce autophagy and brain injury through downregulation the expression of Beclin-1 and LC3.

ABSTRACT

Objective To compare the clinical effect of different administration methods of carboprost tromethamine for the treatment of scar uterus with cesarean resection.Methods 120 cases of puerpera with uterine scar uterus and postpartum hemorrhage tendency were selected,and the history of cesarean section was once and more than once.According to the digital table,they were randomly divided into observation group(60 cases)and control group(60 cases),the observation group was divided into single carboprost tromethamine administration group(30 cases) and sub carboprost tromethamine administration group (30 cases).The clinical efficacy of the three groups were compared.Results In the observation group,the incidence rates of postpartum hemorrhage(13.33%,10.00%)were significantly lower than 28.33% in the control group(χ2 =10.844,6.822,all P 0.05).24h postpartum hemorrhage in the observation group[(263.11 ±137.04)mL,(359.98 ±212.35)mL]were significantly lower than those of the control group[(461.52 ±127.67)mL](t =5.795,2.247,all P 0.05).Conclusion Carbo-prost tromethamine can significantly reduce the bleeding rate of uterine scar again after cesarean section,sub carbo-prost tromethamine administration can significantly reduce the amount of bleeding in postoperative 24h,it is worth popularizing in clinical.

ABSTRACT

Objective To explore the relationship between expression of CD142 protein and the promoter methylation in the placenta of severe preeclampsia patients.Methods We assessed 24 patients complicated with severe preeclampsia as case group and 24 normal pregnant women as control group via qRT-PCR, immunohistochemistry and Western blotting for CD142 expression and MSP technology for methylation in CD142 promoter region.Results The relative expression quantity of CD142 mRNA in severe preeclampsia group (1.45± 0.42)was higher than that in normal group (0.25±0.28)(P <0.05).The expression quantity of CD142 protein in severe preeclampsia group (0.857±0.043)was higher than that in normal group (0.248 ±0.035)(P <0.05).The positive rate of CD142 promoter region methylation was lower in severe preeclampsia group than in normal group (29.2% vs .100.0%,χ2 =36.11,P <0.001)while the positive rate of CD142 promoter region unmethylation was higher than that in the latter group (100.0% vs .20.8%,χ2 =29.85,P <0.001).A negative correlation was found between the CD142 promoter methylation level and the expression quantity of CD142 protein (r = -0.909,P <0.05).Conclusion The expression of CD142 protein regulated by the promoter methylation plays a crucial role in the mechanism of severe preeclampsia.

ABSTRACT

Objective Influenza A(H7N9) virus causes a relatively high mortality in humans and therefore it is of great sig-nificance to know its prevalence in China .This article aimed to study the genetic characteristics and evolution of the hemagglutinin (HA) gene of the influenza A(H7N9) virus prevailing in China between 2013 and 2017. Methods We downloaded the HA se-quences of the influenza A ( H7N9) virus prevailing in China between 2013 and 2017 from The Global Initiative on Sharing All Influen-za Data and National Center for Biotechnology Information .Using the bioinformatics software , we analyzed the homology , molecular characteristics , phyletic evolution , and selective pressure of the HA gene. Results The homology of the HA gene of the influenza A ( H7N9) virus and the reference strain was decreasing each year from 2013 to 2017, 99.0%-99.9%in 2013, 98.7%-99.5%in 2014, 98.4%-99.6%in 2015, 76.8%-99.4%in 2016, and 69.9%-98.2%in 2017.Compared with the reference strain , the HA gene of the influ-enza A(H7N9) virus underwent variations in 21 antigenic sites.The variation of N285D was the highest (23%) in 2015 and that of R148K increased yearly,reaching 65%in 2016 and 78.5% in 2017. Phylogenetic analysis showed a concentrative distribution of the influenza A ( H7N9) virus strains on the phylogenetic tree in the same year from 2013 to 2017.Amino acid substitution of T 140A was observed in most of the influenza A ( H7N9) virus strains from Guang-dong in 2013, and the widest distribution of the virus strains was found in 2014.Positive selective pressure site 65 was obtained in the sequence of 2015 using the FEL and IFEL models, but not in the strains of 2016 or 2017. Conclusion Influenza A(H7N9) virus constantly undergoes variation , which has increased the difficulty in its prevention and control .More importance should be attached to observation of the virus and response to its adaptive mutations .
